In signing this form, the owner gives full permission to the Animal Hospital to publish their photos.  Once the form is signed, it can be emailed, faxed (see our Contact Us tab for information), or delivered in person to the Animal Hospital of Woodstock along with a digital copy of your photo(s).  We will then post them in our gallery.  We are excited to see your pictures!
